Unplug the power cord of the CD player from the AC outlet for 30 seconds. Plug the power cord back into the AC outlet. Turn on the CD player and attempt to play the disc. If the issue is still not resolved, remove the disc and unplug the power cord from the AC outlet. Let the CD player remain without power for 1 hour in case of condensation.
Thanks to FgSanchez and Dkal for help. Just found out from the dealer last week that the part # I have for the DVD (5L3J 10E947 BA3BJA) is only good for F-150's manuafactured betweeen 8/23/04 to 11/28/04. Since my '04 was made earlier, I need the part # 4L3Z 10E947AAA which is good for those F-150's made between 12/1/03 and 8/22/04.
First, turn your system off. Then unfold a paperclip and gently push it into the hole. You may need to repeat this a few times for the mechanism to catch. The drive bay should eject enough for you to grab it with your fingernails. Slowly pull the drive out, but don't force it and risk physically damaging the drive.
To re-enable the DVD player buttons, turn the audio unit off and back on. The headphone symbol lights up on the audio unit display to indicate the DVD player buttons are re-enabled. Ok after a little careful research, I found out I do NOT have the factory DVD unit, it's aftermarket. So the only way I can get sound through the speakers is with a rf to fm converter. PLUS additional headsets are only $15 through Amazon. Thanks for the feedback everyone!

I did have concern about the shelf structure also. The TV/DVD combo is surprisingly light at only 6 lbs. So far, no bouncing at all. I did have the upper shelf structure removed for this install. The shelf do have two attachments at the center of the shelf. The aft center attachment only attaches to the headliner.
